Medical forms and guidance

Please find attached Medical Forms which need completing every school year at the beginning of September and throughout the year if your child's medical or allergy needs change.

The forms below are for Asthma and Allergies.

Please complete them in full and return to us, attaching a recent photo (passport sized for the form) and any medication in original packaging with the child's name on.

Covid 19 Absences

As with any virus, please keep your child off school if they have a high temperature or are too unwell to attend.

Diarrhoea and Vomiting absences

Diarrhoea and/or vomiting commonly affects children and staff and can be caused by a number of different germs. Infections can be easily spread from person to person (by unwashed hands), especially in children.

Children with diarrhoea or vomiting must stay away from school until they have not been sick or had diarrhoea for at least 2 days (48 hours). This is in line with NHS recommendations and helps prevent the spread of illness within the school community.

We appreciate that this is inconvenient in many cases, and you may not believe your child is ill, but you will appreciate that we do this in all cases, and it will reduce the risk of infection for all children in school.
