Medical forms and guidance

Please find attached Medical Forms which need completing every school year at the beginning of September and throughout the year if your child's medical or allergy needs change.

The forms below are for Asthma and Allergies.

Please complete them in full and return to us, attaching a recent photo (passport sized for the form) and any medication in original packaging with the child's name on.

Covid 19 Absences

We ask that parents and cares follow the latest government guidance if their child has Covid 19: please keep your child at home for three days. They can return after three days if they are well enough. They should not return to school if they have a temperature. 

Diarrhoea and Vomiting absences

Diarrhoea and/or vomiting commonly affects children and staff and can be caused by a number of different germs. Infections can be easily spread from person to person (by unwashed hands), especially in children. In general, it is recommended that any child with diarrhoea and/or vomiting symptoms should stay away from the school or early years setting until they have been free of symptoms for 48 hours (the ‘48 hour rule’) and feel well. We appreciate that this is inconvenient in many cases, and you may not believe your child is ill, but you will appreciate that we do this in all cases and it will reduce the risk of infection for all children in school.
